To improve the quality of my sessions and continue developing my skills, I have introduced a consultation form and a post-session questionnaire.
From now on, I plan to keep a short session diary for each participant. I hope these entries will also help women who are considering booking a session understand what the experience may be like.
No identifying personal information will be included. If there is anything you would prefer not to have published, I will leave it out.
This session’s participant was Hitomi, a 26-year-old office worker.
Her reason for applying was:
“Internal stimulation was painful and did not feel pleasurable, so I wanted to see if I could change that.”
Pre-Session Consultation
The following conversation has been recreated based on notes I took during the consultation. No audio recording was made.
Sexual Experience
Have you ever used a jofu service (a women-oriented male escort service) or experienced a sensual massage for women before?
HitomiNo, this will be my first time.
How old were you when you first had sex, and how many people have you had sex with in total?

I first had sex at 23, and I’ve had sex with two or three people.
Are you currently in a relationship?

Yes, I am.
What has sex usually been like for you?

My boyfriend does not seem to enjoy foreplay very much, so he rarely uses his fingers on me or gives me oral sex. At the same time, he seems to expect me to perform oral sex on him as if it were only natural.
I have not felt very satisfied, so I usually pretend to enjoy it. I fake the sounds I make, and when he tries to make me orgasm, he will not move on until he thinks I have. Because of that, I tense or jerk my body and pretend to climax every time.
About Orgasms
Have you ever experienced clitoral, vaginal, or mind-induced orgasms?

I can reach orgasm through clitoral stimulation, but I have never experienced a vaginal or mind-induced orgasm.
How do you feel after a clitoral orgasm? Can you have another one soon afterward?

Immediately afterward, I do not want to be touched anymore. It feels overly sensitive—almost like a stinging sensation, although it is not exactly painful.
I usually recover after a minute or two, and after that, I can sometimes have another clitoral orgasm.
Have you ever reached orgasm from stimulation by a partner?

No, I have never had an orgasm with a partner.
What do you imagine vaginal and mind-induced orgasms feel like?

I imagine a vaginal orgasm might feel weaker than a clitoral one.
Does a mind-induced orgasm mean your brain sort of short-circuits?
I have heard the terms before, but I cannot really imagine what either experience would feel like.
Sexual Preferences
How do you usually masturbate, and how often?

Usually one to three times a week. I tend to do it while imagining my boyfriend touching me. I just lie down and use my fingers—I do not use toys or anything like that. I usually have a clitoral orgasm after about 10 to 20 minutes, and then I stop.
Do you have any particular kinks? What kinds of sexual situations have turned you on in the past?

I would not necessarily call it a kink, but I think I am quite submissive. I also find it a little exciting to do something sexual in a situation where other people are nearby, while trying not to get caught.
Are there any types of play you are interested in or would like to try?

Since I am submissive, I am interested in being tied up. I would prefer something light rather than serious bondage—just having my hands or feet gently restrained. I am also interested in trying sex toys.
Are there any types of play you are uncomfortable with or do not want to try?

Nothing in particular.
How would you describe your dominant-to-submissive balance?

I would say I am about 30% dominant and 70% submissive.
Additional Information
Do you experience pain during intercourse?

The vaginal opening hurts, and the skin at the lower edge seems to tear every time. I do not think my boyfriend is particularly large, though.
It may depend on the position, but it happens every time in the missionary position. Penetration from behind is too painful for me.
Do your clitoris or nipples tend to become painful during stimulation?

My clitoris starts to hurt if it is stimulated for too long. Even when I masturbate, I usually leave the clitoral hood in place rather than pulling it back.
My nipples do not usually become painful.
How sensitive do you consider yourself to be? Are there any areas of your body that are particularly sensitive?

I do not feel much from internal stimulation. However, I think my nipples, ears, and back are on the sensitive side.
Session Details and Impressions
Because she was already fairly sensitive in areas other than inside the vagina, helping her experience both vaginal and mind-induced orgasms went relatively smoothly.
She had mentioned that she was interested in being restrained, so partway through the session, I used bondage tape to gently restrain both of her hands. This may also have helped increase her excitement.
We then tried penetrative sex to help her become more familiar with experiencing vaginal orgasms during penetration. However, she still seemed to feel some pain. I did not want her to push herself and leave with another painful memory, so I kept movement to a minimum and allowed her to experience a vaginal orgasm with almost no thrusting. We stopped after about a minute.
Pain during intercourse can sometimes be reinforced by memories of previous painful experiences. For now, the priority is to help her body become more familiar with pleasurable sensations instead.
There did not appear to be any obvious physical restriction, such as an unusually narrow vaginal opening. Going forward, I think the pain may gradually improve if she is able to have comfortable, pleasurable sex with her boyfriend and avoids intercourse until the area has fully healed whenever any tearing or bleeding occurs.
She had also mentioned that she might be turned on by doing something sexual without the people around her noticing. Before we parted, I suggested that she try remembering the session on the train home and see whether she could experience a mind-induced orgasm discreetly.
Post-Session Questionnaire
What made you decide to book a session?
I decided to try it because I did not usually find internal stimulation very pleasurable.
What was your impression of the therapist?
He was very easy to talk to, and I had a very positive impression of him.
How would you rate your satisfaction with the session on a scale of one to five?
★★★★★
Please feel free to share any comments about the session or a message for the therapist.
I was a little overwhelmed because I experienced so many things for the first time, but I was able to relax throughout the session.
It was a very positive experience. Thank you so much.

Even while Aki-san was speaking softly to me, I could already feel a tingling sensation somewhere around my lower abdomen or vaginal area.
After moving to the bed, I was a little nervous when a toy was inserted because it was my first time. However, it went in much more smoothly than I expected. As soon as it was inside, it seemed to touch a warm, slightly pleasurable spot.
That area gradually became hotter and more sensitive throughout the session, and I realized, “So this is what it feels like to experience pleasure internally.” The most intense moment was when my nipples were being touched while I was also being stimulated internally. It felt incredibly good.
Until now, I had only ever experienced clitoral orgasms. A clitoral orgasm feels like a sharp, concentrated sensation of heat and tension around my clitoris. A vaginal orgasm, by contrast, felt like heavy waves rising from deep inside my body. Each time a wave came over me, my mind seemed to go completely blank.
During the mind-induced orgasms, my sensitivity somehow kept increasing as Aki-san whispered to me. I could not believe how strongly I reacted to something as simple as having my nipples touched or pressure applied to my abdomen.
At first, I wondered whether the sensation meant I was about to orgasm. Before I knew it, I was clearly having one. My entire body began trembling, and eventually I could no longer think about anything.
The sensations have not completely faded yet. Even now that I am home, I can still feel a tingling sensation deep in my lower abdomen.
Sex had always involved pain for me in the past, so this experience made me realize all over again just how much pleasure my body is actually capable of feeling. It was a truly wonderful experience.
I had always held part experience.
I had always held part of myself back before, but today I completely let go. I naturally found myself crying out things like “That feels so good” and “I’m going to come”—things I would never normally say out loud.
That is how amazing it felt.
Thank you so much! 🙇
